FedEx Release Signature Form – Leave Packages at Door without Signature

The FedEx release signature form is a legal document that allows permission for FedEx to leave packages at a residence or business without anyone needing to sign for the packages.

Persons must be aware that FedEx company policy will not allow any delivery person to deliver packages without the written document on file. Another option, if the person(s) prefer not to make the document available, would be to have someone near you (ie: a close neighbor or a neighboring business) may accept the package by signing for it on behalf of the recipient, however, the document must be submitted to FedEx indicating another address.

It is also company policy that the driver have the right to deliver at their discretion for the purpose of their personal security as well as the security of FedEx property and client’s loaded property.

Notes.
FedEx door tag tracking number start with the letters “DT” followed by 12 numbers.
FedEx direct signature release required: FedEx obtains a signature from someone at the delivery address.

How to Write

Step 1 – Download in Adobe PDF (.pdf).

Step 2 – Begin by printing the document for use if the recipient will not be available on the day of delivery.

  • Mark the preferred location for delivery
  • The package will not be left if there is no valid signature
  • One document must be completed for each package to be delivered

Step 3 – Authorization –

  • Submit the tracking number
  • Recipient’s name
  • Recipient’s complete address
  • Telephone number
  • Zip Code
  • AND
  • When the package arrives, check the designated location from the boxed selections provided
  • Enter the address, if the package will be delivered to a neighbor

Step 4 – Signature –

  • The actual recipient, must read the statement of authorization and release of liability to FedEx
  • If in agreement – Print the first and last name of the recipient
  • Submit the recipient’s signature
  • Date the signature in mm/dd/yyyy format
  • The remainder of the document is for FedEx company use only
  • The recipient must then affix the document to the front door of the residence or business
